Position Description
SUMMARY DESCRIPTION OF WORK

  • Intense case management responsibilities begin at this level. Decision making, problem solving and highly developed interpersonal skills are critical. Professionals at this level are expected to work with a minimum of supervision. High production and quality are required.
PREFERRED MINIMUM QUALIFICATIONS
  • Bachelors Degree and 2 years experience, including working with child abuse victims or
  • Masters Degree and 1 year experience.
  • Valid drivers license/ acceptable driving record.
ACCOUNTABILITIES
  • Provides in-home service, training and support in developing and utilizing functional skills to individuals/families with problems, i.e., child management, personal and family adjustments, finances, employment, and physical and mental impairments.
  • Assesses problems, prepares proper documentation, and treatment plans.
  • Counsels with family members and monitors individual/family progress toward reaching predetermined goals; provides for regular review and updating of treatment plans. Develops and maintains effective working relationships with families and builds on families/ competencies.
  • Maintains regular on-call status for program support
  • Provides training in the management of difficult behaviors as needed to program consumers, including school personnel and others working with children served by the program.
  • Develops and maintains effective working relationships with necessary support services for the program.
  • Develops intervention strategies with therapist for families as needed.
  • Completes all case documentation and is prompt and accurate in completing all required reports, statistics and other requirements.
  • Is dependable and punctual regarding scheduling and attendance.
  • Displays a courteous and caring attitude at all times to the clientele, volunteers, and visitors of the Agency.
  • Cooperates and collaborates with agency and collateral staff.
  • Abides by all specific program and agency procedures, policies, and other requirements.
  • Develops personal and program related skills through participation in internal and external training opportunities including training sessions, printed material and audio and/or visual media.
  • Performs other program related duties as assigned
